To process your request we must verify your identity. We may ask for contact, account, or health information. You may designate an authorized agent to act on your behalf; written permission is required, and we may ask you to verify your identity directly. Only a parent or legal guardian may submit a request on behalf of a child under 13. You may submit two requests to know or delete in a 12-month period.
We will confirm receipt of your request within 10 business days, explain our verification process, and provide a timeline for response.
Changes to This Notice
We may change this Notice at any time. We will not use personal information already collected for a materially different purpose without obtaining your explicit consent.
