Important: ForHumanity Health is not your covered entity for care
ForHumanity Health, Inc. operates a consumer health platform (BrandCo / platform model). Independent medical groups and licensed clinicians deliver care and make prescribing decisions. Independent pharmacies compound, dispense, and ship medications. Those partners may be HIPAA-covered entities and publish their own Notices of Privacy Practices (NPPs). ForHumanity Health is not a medical group or pharmacy and is not a HIPAA-covered entity for Platform and advertising data described in our Privacy Policy.
If you receive telehealth care or pharmacy fulfillment through partners coordinated via the Platform, your clinical protected health information (PHI) is governed by the applicable partner's Notice of Privacy Practices and by HIPAA as it applies to that partner — not by a ForHumanity Health covered-entity NPP.
Partner NPPs are provided by the medical group or pharmacy at or before care, and may also be linked from intake or portal surfaces when you are matched to a specific partner. If you need a copy of a partner NPP, contact support at privacy@forhumanity.co and we will help route your request to the correct partner.
How ForHumanity Health collects, uses, and shares Platform information — including limited non-PHI commercial signals for advertising conversion measurement after you accept Terms + Privacy — is described in our Privacy Policy. Related platform terms are in our Terms & Conditions.
We do not send diagnosis, medications, lab results, email, phone number, name, or other PHI (or hashes derived from PHI) to advertising networks for conversion measurement. See the Advertising & Conversion Measurement section of the Privacy Policy.
