What is the difference between total and free testosterone?
Total testosterone measures all of the testosterone circulating in your blood. Most is bound to proteins, primarily SHBG and albumin, while a small percentage circulates unbound as free testosterone. Free testosterone measures this unbound fraction.
What is SHBG, and why does it matter?
SHBG, or sex hormone-binding globulin, is a protein produced primarily by the liver that binds testosterone tightly. Albumin also binds testosterone, but much more loosely. Because SHBG levels vary from person to person, they can affect the relationship between total and free testosterone.
Why does free testosterone add context that total testosterone alone does not?
Total testosterone is generally the starting point for evaluating testosterone levels. Free testosterone can provide additional information when total testosterone is borderline or when SHBG levels may be affecting how the total testosterone result should be interpreted. This is why clinicians look at laboratory values in context rather than relying on one number alone.
If my testosterone is technically low, does that mean I need TRT?
Not automatically. A diagnosis of testosterone deficiency is based on symptoms or signs consistent with testosterone deficiency together with consistently low testosterone levels. Because testosterone levels vary, diagnosis generally requires at least two appropriately obtained morning testosterone measurements rather than a single low result.
What else factors into whether treatment makes sense?
Your symptoms, medical history, medications, other laboratory findings, and potentially reversible contributors all help determine what a testosterone result means. LH and FSH can also help identify whether low testosterone originates primarily in the testes or higher in the hormonal signaling pathway.
Can a “normal” testosterone result still require a closer look?
Sometimes. Reference ranges need to be interpreted in clinical context, and total testosterone does not always provide the complete picture. When total testosterone is borderline or SHBG is altered, free testosterone may provide useful additional information. Symptoms also matter, although symptoms alone cannot diagnose testosterone deficiency.
Sometimes, yes. Before assuming TRT is the only option, it is important to consider whether something potentially reversible is contributing to a low testosterone level.

Exercise remains important for cardiovascular health, metabolic health, muscle mass, body composition, and overall health, regardless of whether it produces a meaningful increase in resting testosterone.
None of this means TRT is unnecessary when it is genuinely indicated. It means that identifying why testosterone is low is part of deciding how best to treat it.
Testosterone production does not start in the testes. It starts in the brain, through a feedback system called the hypothalamic-pituitary-gonadal (HPG) axis.
Hypothalamus → Pituitary → LH/FSH → Testes → Feedback to the Brain
The hypothalamus signals the pituitary gland, which releases luteinizing hormone (LH) and follicle-stimulating hormone (FSH). Those hormones travel to the testes and instruct them to produce testosterone and sperm. As testosterone rises, the brain detects that level and adjusts its own signal down, a feedback loop that keeps the system in balance under normal conditions.
This is also why LH and FSH are checked alongside testosterone rather than skipped. High LH and FSH with low testosterone points to the testes not responding to a strong signal. Low or inappropriately normal LH and FSH with low testosterone points to a weak signal from the brain itself, which sends the evaluation in a different direction entirely.
